Information for Patients

Thank you for choosing Houston Institute for Sports Medicine and Orthopedics for your health care needs. We are excited to help you eliminate pain and regain your mobility. The information below is intended to answer general questions about our orthopedic practice and covers information about appointments, medication, insurance, and more. If you have any other questions, please feel free to call our offices at (713) 756-5546.

To schedule, re-schedule, or cancel an appointment, please call (713) 756-5546. If, for any reason, you cannot keep your appointment, please notify the office at least 24 hours in advance. Failure to do so will result in a $25 fee.

Please bring along the following information to your appointment:

  • Insurance card(s)
  • Government-issued photo identification
  • If your address is different from what appears on your government-issued photo identification, please bring a copy of a current utility bill with your name and current address.
  • Payment for services
  • Complete list or actual bottle of the names and dosage of all the medications you take
  • A list of questions you want to ask your doctor

In order to reduce wait times and serve our patients as quickly as possible, we ask that you download and fill out the following form prior to your first visit. Please bring your photo ID and insurance cards, as well.

Medications & Refills

Please allow one full business day for medication refill requests to be fulfilled. If you regularly take a prescription medicine, then your physician may ask to see you periodically.

Insurance

Houston Institute for Sports Medicine and Orthopedics accepts most major insurance plans. To find out if your plan is accepted, please call us at (713) 756-5546.
